Hypertension Diabetes mellitus Treated or detected persistent hypertension from previous consultations (BP >140/90 mm Hg)
Treated or detected fasting glucose ≥126 mg/dL (7.0 mmol/L), or plasma glucose ≥200 mg/dL (11.1 mmol/L) after glucose load in the oral glucose tolerance test, or glycated hemoglobin ≥6.5%

Hyperlipidemia Treated or detected total cholesterol ≥200 mg/dL (5.2 mmol/L), or LDL cholesterol ≥130 mg/dL (3.4 mmol/L)

Ischemic heart disease History of angina pectoris, myocardial infarction, or evidence of coronary artery disease from angiography

Congestive heart failure Clinical history of cardiac failure or echocardiography with ejection fraction <45%
