68-year-old female with known unresectable pancreatic adenocarcinoma located within the pancreatic head presented with elevated bilirubin levels and medically unresponsive pruritus. (b) MRCP image shows dilation of the proximal intra- and extrahepatic bile ducts (arrowheads). (c) Percutaneous biliary drainage was performed through right liver lobe and the occlusion caused by the mass (arrow) and a bare metal stent was implanted (d) Follow-up cholangiography from the safety catheter showed satisfactory flow through the stent.
