Cephalic orientation of anterior ribs (arrows) indicates X-ray tube misalignment. Apparent enlargement of the cardiac silhouette due to oblique orientation of the radiograph towards the left and prominent thymic shadow, which is causing superior mediastinal widening (dotted arrow) as seen on the lateral radiograph.
