*Scoring the Trust in CHWs Scale. Responses are scored as follows: 1 (never), 2 (some of the time), 3 (most of the time), 4 (all of the time). Generate mean sub-scale and full-scale scores by taking the mean of non-missing items for a final range of 1 to 4.
