Recommendation for sentinel lymph nodes

The presence of isolated tumor cells (ITCs) in lymph nodes should be recorded but this does NOT count as nodal involvement and should not result in tumor upstaging The number of lymph nodes harboring ITCs/micro/macrometastases should be recorded When ITCs are scattered throughout the lymph node, measure only contiguous tumor cells to designate ITCs and not aggregate all scattered clusters into a single measurement; however, if multiple collections of ITCs are present, this should be documented in the pathology report. When there is lymph node involvement, the presence or absence of extracapsular spread should be documented.
