Recommendation for sentinel lymph nodes
  • The presence of isolated tumor cells (ITCs) in lymph nodes should be recorded but this does NOT count as nodal involvement and should not result in tumor upstaging

  • The number of lymph nodes harboring ITCs/micro/macrometastases should be recorded

  • When ITCs are scattered throughout the lymph node, measure only contiguous tumor cells to designate ITCs and not aggregate all scattered clusters into a single measurement; however, if multiple collections of ITCs are present, this should be documented in the pathology report.

  • When there is lymph node involvement, the presence or absence of extracapsular spread should be documented.
