Green indicates that the patient either complied with the corresponding instruction (columns 1-5) or did not have the corresponding symptom (columns 6-13), orange indicates that the patient gave a positive answer to the corresponding mild heart failure (HF) symptom question, and red indicates that the patient gave a positive answer to the corresponding moderate or severe HF symptom question. Gray indicates skip logic, meaning these questions were not required based on the previous answers.
