Clinic provided data. South Clinic collected surveys during clinic visits; consent was in English decreasing Hispanic/Latino respondents, but there were no other exclusions. West Clinic collected data through an electronic medical record search for patients with T1D with two visits in year prior to study. Both clinics have additional URM patients with type 2 diabetes.
