 diagnosis: probable Alzheimer's disease NINCDS‐ADRDA criteria 
 mild to moderate dementia 
 MMSE 10‐26 mean 19 (5) 
 Exclusion : 
 uncontrolled hypertension 
 cardiac disease 
 stroke 
 renal insufficiancy 
 seizure 
 syncope 
 alcohol or drug abuse 
 active skin disease 
 sensitivity to medical dressings 
 major depression 
 tacrine or donepezil 
 liver disease 
 Parkinson's disease
Interventions 1. placebo patch 
 2. nicotine patch (Nicotrol) (5mg/day for 1 week , 10mg/day for 2 weeks, 5mg/day for the 4th week)
