Interventions |
First component is nurse‐led case management including 4 elements: 1) evaluation of participant needs and resources; 2) establishment and maintenance of a patient‐centred individualised service plan; 3) co‐ordination of services among partners; and 4) self‐management support for participants and families. Second component includes group meetings (10 to 12 participants) for self‐management support in accordance with the Stanford programme. |
