Study name	Mount Sinai Palliative care at home for dementia
Methods	Randomised trial, parallel assignment, single blind (outcomes assessor)
Participants	Adults aged ≥ 65 years, residing in the catchment area, diagnosed with advanced dementia
Interventions	Intervention group: interdisciplinary team including social worker, community health worker, nurse practitioner and physician Control group: usual care
Outcomes	Main outcome: dementia symptoms Other outcomes: quality of life, healthcare services use, caregiver burden and quality of life
