Interventions Furosemide 40 mg then:
Continuous: 250 mg in 250 cc of 5% dextrose water started at 0.1 mg/kg/hour to titrate to hourly urine output >/= 1 cc/kg
Bolus: Repeat or double previous dose to net hourly urine output >/= 1 mg/kg
