Proposed clinical decision algorithm vaccination in sarcoidosis with or without prior COVID-19 infection. Recommendations are based on ACR guidance for COVID-19 vaccination in patients with rheumatic and musculoskeletal diseases (32) RTX, rituximab; TNF, tumor necrosis factor. *Current guidance recommends no adjustment in patients on anti-TNF alpha therapy. However, recent data suggests that in some patients on infliximab, the antibody response to SARS-CoV-2 may be blunted (33). At this time, it is unknown if this is applicable to sarcoidosis patients.
