Inclusion and Exclusion Criteria

Inclusion criteria

1. Patients discharged from hospital at least 1 mo before the review 2. Age group 21–90 y 3. Latest admission period >24 h and <90 d

Exclusion criteria

1. Incomplete case records without discharge summary or discharge coding 2. Duration of admission period <24 h or >90 d 3. Patient is admitted to hospital at the time of collection of patient record.
