Capsular reconstruction begins with the placement of sutures circumferentially around the capsular rent in a horizontal mattress fashion. If the acetabular-sided capsule is absent, suture anchors are placed approximately 5 mm apart to fix the graft to the acetabulum. As shown from the anterolateral portal (superior view using a 70° scope), acetabular-sided leaflet sutures are placed in the healthy acetabular-sided capsule, prior to anterosuperior capsular reconstruction, in a right hip (A), and acetabular-sided suture anchors are placed in the acetabular rim in the setting of acetabular-sided capsular deficiency in a left hip (B).
