Steps of Arthroscopic Capsular Reconstruction With Acellular Dermal Allograft

  • 1.

    Enter the hip through the previous capsular rent.

  • 2.

    Perform takedown of the capsular scar and identify the competent capsule.

  • 3.

    Identify whether the capsule is intact on the acetabular side. If not, use suture anchors to fix the graft on the acetabular side.

  • 4.

    If there is a capsular defect or if the capsule apposition appears over-tensioned, consider performing a capsular reconstruction.

  • 5.

    Place sutures around the capsular periphery.

  • 6.

    Place sutures sequentially through the graft, paying attention to meticulous suture management.

  • 7.

    Parachute the graft down the cannula and tie into position.
