Reimbursement rates for private and statutory health insurance were extracted from the respective reimbursement catalog: “Gebührenordnung für Ärzte (GOÄ)” and “Einheitlicher Bewertungsmaßstab (EBM)” (Brück 1998; Kassenärtzliche Bundesvereinigung 2000, 2008, 2015). Time consumption for physicians, medical assistants, and patients was extracted from relevant literature and confirmed in expert interviews
