Unfractionated heparin (UFH) can be used in all patients. I A UFH is preferential in patients with severe renal dysfunction (< 15 mL/min) and obese patients (weighing > 150 kg). IIa B
Low-molecular-weight heparins (LMWH) can be used in all patients. I A Enoxaparin can be used in patients without severe renal dysfunction (creatinine clearance < 15 mL/min/1.73 m²), until revascularization, for 8 days, or until hospital discharge, 1 mg/kg every 12 h (0.75 mg/kg, every 12 h, if ≥ 75 years old; use 1 mg/kg every 24 h if creatinine clearance is between 15 and 30 mL/min/1.73 m², with a maximum dose of 150 mg.
