Sensitivity analysis 1 conducted for patients who completed the PROMIS GH in both August 2019 and August 2020 in the same medical centers. Models adjusted for all variables in August 2020 listed in Table 1; sensitivity analysis 2 conducted for patients who completed the PROMIS GH via MyChart in August 2019 or August 2020. Models adjusted for all variables listed in Table 1 (except MyChart completion).
