Fluid resuscitation, intubation and vasopressors prior to transfer. Empiric ceftriaxone and vancomycin were administered after blood cultures. Ethanol infusion was started then discontinued when the anion gap became normal. The patient improved with mechanical ventilation, vasopressor support, ceftriaxone, corticosteroids and supportive ICU care.
