Full thickness skin loss Adipose tissue exposed. Muscle, tendon or bone are not exposed. If slough or eschar obscures extent of tissue loss, this is Unstageable Pressure Injury |
| Stage 4 |
Full thickness skin and tissue loss Exposed or directly palpable fascia, muscle, cartilage, tendon or bone. |
| Unstageable Pressure Injury |
Obscured full-thickness skin and tissue loss Tissue loss in which extent of damage cannot be confirmed due to slough/eschar. When removed stage 3 or 4 injury will be revealed |
| Deep Tissue Injury |
Persistent non-blanchable deep red, maroon or purple discolouration Persistent sheath/friction at bone-muscle interface resulting in intact, discoloured skin. May rapidly evolve to reveal actual extent of tissue injury |
