A 58-year-old male with portal hypertension who was undergoing TIPS placement for refractory ascites. Portal venogram demonstrated a brisk capsular bleed (black arrow). Despite best medical management in the ICU and repeated transfusions the patient subsequently expired. TIPS, transjugular intrahepatic portosystemic shunt.
