The superior retractor is at the angle of the mandible, the left retractor is reflecting the sternocleidomastoid (SCM), the right retractor is reflecting the mobilized internal jugular vein, and the Penrose drain is encircling the omohyoid muscle. All of the nodal tissue beneath the SCM and around the internal jugular vein to the carotid has been removed. The Phrenic vein is visible running along the anterior scalene muscle.
