Population: adults with depression, COPD, T2DM, arthritis or coronary heart disease Baseline characteristics: NR Inclusion criteria: people attending primary care aged ≥ 50 years, ≥ 2 of 5 common conditions, fluent in written English Exclusion criteria: unable to consent, or considered unsuitable by the primary care team |
