No Hlth Ins is defined as a response of “No” healthcare coverage, including health insurance, prepaid plans such as HMOs, or government plans such as Medicare or the Indian Health Service. No Provider is defined as a response of “No” when asked if there was a person the respondent could identify as a personal doctor or healthcare provider. Cost Prev Access is defined as a response of “Yes” when asked if there was a time in the past 12 months when the respondent needed to see a doctor but could not because of cost. No check-up is defined as not visiting a doctor for a routine check-up within the past 12 months. No flu shot is defined as not receiving a flu shot or flu vaccine during the past 12 months. No Pneu Vac is defined as never receiving a pneumococcal vaccine.
