Coil embolization of a falciform artery. A 64-year-old man with hepatitis C virus cirrhosis and CT presented with a 3-cm subcapsular hepatocellular carcinoma (HCC) abutting the gallbladder ( a ). With plans for Y-90 radiation segmentectomy as a bridge to liver transplant, he underwent mapping angiography. Left hepatic arteriography shows tumor vascularity (asterisk) partially supplied by the falciform artery (arrow) ( b ). Selective arteriography before ( c ) and after ( d ) coil embolization (black arrow) of the falciform artery. (Arrowhead = tumor vascularity). Bremsstrahlung SPECT/CT shows expected focal deposition of Y-90 microspheres in the HCC and no nontarget Y-90 deposition. The patient subsequently underwent liver transplant.
