Inclusion and exclusion criteria.

Inclusion Criteria	People diagnosed with COPD, HF or T2DM by a General Practitioner, pneumologist, endocrinologist or cardiologist
	Native Spanish-speaking population
	People who were able to read and understand questionnaires properly
	People who were able to provide informed consent
Exclusion Criteria	People with cognitive deterioration, mental disorders or pharmacological side-effects that could potentially disrupt the assessment
