Insufficient acid suppression

Reduced bioavailability of PPIs
Hypersecretory state (eg, Zollinger-Ellison syndrome)
Weakly acidic or non-acidic reflux
Concomitant functional disorder or psychological comorbidity
Delayed gastric emptying
Reflux hypersensitivity
