We think of dementia beginning as sub-clinical white matter alteration that can be detected by MRI and progressing through two pathways, Alzheimer's and vascular disease, independently or concurrently. Some patients, with mixed dementia exhibit features of both pathways. In this paper, we do not measure progression over time but we do divide dementia into four categories.
