Representative case of parotid gland adenoid cystic carcinoma treated with defRT using IMRT. The 75-year-old woman with left parotid gland cancer, cT4bN0M0. Left facial nerve disorder was observed. (a) MRI contrast-enhanced T1-weighted images revealed the parotid gland tumor with the extension to base of skull. The patient refused surgery and hoped to receive defRT. (b) Dose distribution of defRT using 66 Gy in 33 fractions. The GTV, CTV, and PTV are shown in red, blue, and magenta, respectively. CTV margin was extended to base of the skull considering perineural invasion. PTV margin was added to CTV to account for patient motion and the field margins.
