(B) Instrument is undergoing an aspiration maneuver but is sucking in the vessel wall, potentially producing a false negative aspiration test. (C) Instrument has entered the vessel, but with the pressure of aspiration the vessel wall may be collapsed, blocking the opening and again leading to a false-negative aspiration.
