After radiation treatment, an 80-year-old patient with regional recurrence of previous breast cancer in the left axillary region showed promising results. However, increasing pain in the left arm, forearm, and hand leads to clinical suspicion of recurrence. (a) Initial MRI with TSE Spectral Attenuated Inversion Recovery (SPAIR) T1-weighted imaging with iv contrast before radiation showed an irregular mass in the left axillary cavity involving the BP (curved arrow) and axillary vessels (straight arrow). (b) The pain decreased, and a good effect of radiation was seen after six months on the T1-weighted TSE SPAIR MRI with i.v. contrast. One year later, the patient presented with increasing diffuse pain. The new MRI revealed radiation fibrosis (straight arrow) without the recurrence of the metastasis.
