Cardiac positron emission tomography/computed tomography (PET/CT) with ¹³N-ammonia (top row) and ¹⁸F-flurodeoxyglycose (FDG) (bottom row) demonstrates a perfusion defect involving the basal septum with corresponding abnormal FDG uptake consistent with an inflammatory process. This patient had biopsy-proven systemic sarcoidosis, with history of complete heart block requiring pacemaker implantation. Findings were therefore felt to be consistent with probable cardiac sarcoidosis. In addition, patient was noted to have abnormal FDG uptake in the right ventricular free wall (black arrows), which portends a worse prognosis.
