An urgent care HF visit is defined as an event that meets ALL of the following criteria |

The patient has an urgent, unscheduled office/practice or emergency department visit for a primary diagnosis of HF that does not meet the criteria for a HF hospitalisation |
The patient meets all signs and symptoms, laboratory or diagnostic evidence for HF hospitalisation as indicated in Table 1 |
The patient receives at least ONE of the following treatments specifically for HF: a. Initiation of intravenous diuretic or vasoactive agent (e.g. inotrope, vasopressor, or vasodilator) b. Mechanical or surgical intervention, including: i. Mechanical circulatory support (e.g. IABP, ventricular assist device, ECMO, total artificial heart) ii. Mechanical fluid removal (e.g. ultrafiltration, haemofiltration, dialysis) |
