  • Synthesis of several case reports of sudden onset of amnesia following drug use that occurred 2012–2017

  • 63-year-old pain patient found unconscious after using 3 fentanyl pain patches; regained consciousness after naloxone infusion and toxicology positive for opiates; patient had profound anterograde amnesia

  • Testing ruled out herpes simplex virus, arbovirus & syphilis

  • MoCA score was 19/30 & MRI on day 7 revealed bilateral hyperintensities in hippocampus

  • Day 12 MRI showed resolution of hyperintensities and NP results suggested mild executive impairment

  • Follow-up telephone interview with patient’s husband confirmed persist amnesia

  • One patient initially had non-specific neuroimaging, but on day 13 experienced fluctuating consciousness & MRI revealed bilateral globus pallidus injury; at discharge patient was amnestic, lack of attention & dysexecutive abnormalities

  • Second patient had been treated for HIV and his initial CT scan revealed decreased density in globus pallidus; then after a few days he deteriorated & MRI revealed bilateral lesions in the globus palldus; patient did not regain consciousness
