Study Design Flow Chart

Patients admitted to the cardiovascular unit will be screened for eligibility based on their EFT score, age, and anticipated discharge date. Those who consent will undergo baseline assessments and be randomized to the intervention or usual care group, which they will receive throughout their hospital stay.
