Frail older patients are at risk of hospital-acquired disability and poor quality-of-life after a hospitalization. The interaction of hospitalization-related stressors and pre-existing frailty deficits negatively impacts the capacity for recovery after an acute illness. The TARGET-EFT interventions aim to counteract hospitalization-related stressors and frailty deficits to improve patient-centered outcomes.
