A 70-year-old patient with biochemical recurrence after radical prostatectomy (Gleason 3 + 4 = 7, PSA-nadir < 0.1 ng/mL) with a PSA of 0.7 ng/mL at the PET/CT scan time. Restaging 18F-DCFPyL PET/CT detected multiple bone metastases (>10) at low serum PSA value ((A); maximum intensity projection). Transversal 18F-DCFPyL PET (B,E) and fused PET/CT (D,G) images illustrate two bone metastases (os pubis left, red arrow: SUVmax: 9.76; L5 vertebra, blue arrow SUVmax: 8.02) with sclerotic substrate on CT (C,F,H).
