Constellation of symptoms checklist for cardiac ATTR amyloidosis. Healthcare practitioners should evaluate patients with heart failure with preserved ejection fraction for a clinical history of carpal tunnel syndrome or lumbar spinal stenosis, along with progressive neuropathy or autonomic dysfunction.
