bTSL = Transition Support Level score. The TSL is the percent risk for the study composite outcome (non-elective rehospitalization or death within 30 days of discharge). In this health system, patients with a score of ≥ 25% are automatically enrolled in the program’s follow-up protocols; patients with a score of ≥ 45% receive more intensive follow-up.
