Findings suggestive of pulmonary hypertension in chronic lung disease

Pulmonary function testing • DLCO < 30% or worsening DLCO in the setting of preserved lung volumes
6-Minute Walk Test • Marked or worsening exertional desaturation
• Severely reduced or worsening 6MWT distance
• Impaired (decreased) heart rate recovery following exercise (< 13 beats per minute)
CT scan • Increased pulmonary artery to aorta ratio (> 0.9)
• RV to LV ratio > 1
Echocardiogram • RVSP elevation > 45
• RV dilation
• TAPSE < 1.8cm
• RVOT diameter > 3.4cm
• Reduced RV fractional area change
• Reduced RV ejection fraction on 3D ECHO
Laboratory studies • Elevated BNP
