* Kaplan-Meier survival analysis was used to estimate vaccination coverage based on the month and year of first dose receipt; estimates reflect the cumulative percentage vaccinated as of the end of each month.

Persons were categorized into mutually exclusive categories of race and ethnicity; persons who did not identify as Hispanic were categorized by their reported race or races.

Non-MSA areas include urban populations not located within an MSA as well as completely rural areas.
