Model 2: Model 1 + current smoker, BMI, diabetes, hypertension, CVD history, log-transformed triglyceride, log-transformed HDL-C, statin use or not, prealbumin, log-transformed high-sensitivity C-reactive protein and log-transformed urinary albumin/creatinine ratio. Model 3: Model 2 + categories of estimated glomerular filtration rate.
