Estrogens Around the age of 11 or 12 years 17-beta-estradiol: 0.2 mg/day
(or 0.5 mg every other day) administration either oral or, preferably, transcutaneous: percutaneous delivery via gel or ¼ of a patch delivering 25 µg/24 h to be worn for 10–12 h at night
After 12–24 months 17-beta-estradiol: 0.5 mg/d, administration oral or, preferably, transcutaneous: percutaneous delivery via gel or ½ a patch delivering 25 µg/24 h to be worn for 10–12 h at night
When growth velocity < 2 cm/year: progressive or direct increase in dose Increase doses of 17-beta-estradiol up to 2 mg/day orally or in gel form (1 to 1.5 mg/day) or by patch at 50–75 µg/24 h or 2 patches per week via the transcutaneous route
 + progesterone/progestogens (*) Start when the estradiol dose reaches 0.8 to 1 mg/day or in case of metrorraghia Minimum 12 days per month if discontinuous estrogen treatment and 14 days per month or every day if continuous treatment
