Data are collected in selected counties in California, Colorado, Connecticut, Georgia, Iowa, Maryland, Michigan, Minnesota, New Mexico, New York, Ohio, Oregon, Tennessee, and Utah. A list of these counties is available at https://www.cdc.gov/mmwr/volumes/69/wr/mm6915e3.htm. Iowa did not provide immunization data but is included in the overall population-based hospitalization rates.
