Timing for de-escalating antiplatelet therapy after ACS undergoing PCI. A guided de-escalation strategy allows for an early (days or weeks) de-escalation of antiplatelet therapy while unguided de-escalation is to be performed at least 1 month after ACS and shortening of DAPT is to be performed 1–6 months after ACS, depending on modality (aspirin or P2Y12 inhibitor monotherapy). ACS, acute coronary syndrome; DAPT, dual antiplatelet therapy; PCI, percutaneous coronary intervention.
