Ppa: pulmonary artery pressure; Ppcw: pulmonary capillary wedge pressure; PVR: pulmonary vascular resistance. #: a transpulmonary pressure gradient >12 mmHg could substitute this criterion in patients with an elevated mean Ppcw but fulfilling other criteria; : a PVR >120 dyn·s·cm−5 is recommended by some authors.
