Lichen simplex chronicus of the posterior scalp shows irregular acanthosis with hypergranulosis and pronounced hyperkeratosis reminiscent of the acral skin. There is an increased number of fibroblasts in the upper dermis, and mild perivascular lymphocytic infiltrate (haematoxylin and eosin, ×10).
