I ICD implantation is indicated in patients with HCM who are survivors of SCA or have spontaneous sustained VT. B-NR 46 70
IIa For children with HCM who have ≥1 primary risk factors, including unexplained syncope, massive left ventricular hypertrophy, nonsustained VT, or family history of early HCM-related SCD. ICD placement is reasonable after considering the potential complications of long-term ICD placement. B-NR 71 72
IIb ICD implantation may be considered in patients with HCM without the above risk factors but with secondary risk factors for SCA such extensive LGE on cardiac MRI or systolic dysfunction. C-LD
III Harm ICD implantation is not indicated in patients with an identified HCM genotype in the absence of known pediatric SCA risk factors. C-LD
