SD = standard deviation, SNF = skilled nursing facility, E&M = evaluation and management visits. Observations in Hospital Service Areas below 25th percentile of ACO penetration excluded. Mental Health follow-up measures reflect proportion of index hospitalizations that result in mental health visit with 7 or 30 days of discharge. Attribution to Medicare ACOs based on plurality of E&M visits.
