Clonal BM plasma cells > 10% or biopsy-proven bony/extramedullary plasmacytoma | |||

CRAB | Evidence of end organ damage attributed to underlying plasma cell proliferative disorder | • Hyper-calcemia: serum Ca > 11 mg/dL or serum Ca > 1 mg/dL higher than upper limit of normal • Renal insufficiency: Cr clearance < 40 mL/min or serum Cr > 2 mg/dL • Anemia: Hb value < 100g/L or Hb > 20g/L below the lowest limit of normal • Bone lesions: ≥ 1 osteolytic lesion on skeletal radiography, CT, or PET/CT. If < 10% clonal plasma cells in BM, ≥ 1 bone lesion is required (distinguish from solitary plasmacytoma with minimal BM involvement) |

MDEs | Any 1 of the following biomarkers of malignancy | • Clonal plasma cells ≥ 60% on BM examination • Serum involved/uninvolved free light chain ratio ≥ 100, provided the absolute level of the involved light chain ≥ 100 mg/L • More than one focal lesion on MRI ≥ 5 mm |
